The purpose of this study is to determine if information provided by cancer survivors before their first visit to a survivorship clinic is useful to them and to their clinicians for improving the quality of survivorship care. Participants in this study will complete a confidential online survey before their first survivorship clinic visit which will ask them about their general health and well-being, side effects of treatment, physical and mental function, health behaviors, psychosocial concerns, and access to medical care.
Researchers hope this study will provide a better understanding of survivors’ needs and concerns and the best ways to meet those needs during visits to the survivorship clinic.
Participants in this study include adults who completed treatment for prostate or colorectal cancer at least one month before entering the study.
