Our patient information letter includes some frequently asked questions about scheduling the initial appointment, insurance coverage and concerns, and a brief overview of the Center's privacy policy. It may help guide you in your decision to contact us for an appointment.
Now that you have read our Patient Information Letter, you understand that each patient has to complete a family history questionnaire form, which we will use to create your family tree, or pedigree, prior to receiving an appointment.
