At Memorial Sloan-Kettering, we are dedicated to helping cancer survivors live their lives to the fullest after treatment. We offer comprehensive follow-up care in our disease-specific adult survivorship clinics for many patients who have been treated here.
While we anticipate that most cancer survivors will lead healthy, active lives, some may develop problems as a result of their past diagnosis and therapy. In addition to monitoring for signs of cancer recurrence, we focus on identifying, preventing, and controlling any long-term and late effects associated with cancer and its treatment. We currently provide services for survivors of breast, cervical, colorectal, endometrial, esophageal, kidney, lung, melanoma, ovarian, prostate and thyroid cancers, as well as those who have undergone blood and marrow stem cell transplantation.
At Memorial Sloan-Kettering, your follow-up care is provided by nurse practitioners or physician assistants who are specialists in the care of cancer survivors. Each nurse practitioner or physician assistant works with a collaborating physician who provides mentorship and guidance.
A follow-up visit typically includes:
- Review of your recent medical history and a physical examination
- Assessment to detect recurrence of cancer
- Identification and management of the effects of cancer and its treatment
- Screening referrals for other cancers
- Health promotion recommendations related to nutrition, exercise, and smoking cessation
- Treatment summary and follow-up care plan
- Consultation with your Memorial Sloan-Kettering physician as needed
In addition to working closely with your treatment team, the follow-up care provider gives a treatment summary and survivorship care plan to you, your primary care doctor, and other specialized medical providers. Your physicians can then incorporate this information into your overall medical plan.
