For Medical Students: Dean's Letter

Upon submission of the online application, you will receive an e-mail confirmation with this link providing access to the Dean’s letter template below. Submit your Dean’s Letter together with your official transcript, $100 application fee, and Memorial Sloan-Kettering Health Statement. Applications will not be reviewed until all these items are received.

Dean's Letter of Recommendation must contain the following information on medical school letterhead:

  • Student Name
  • Student is in good standing at the school (include school's name).
  • Student is in 4th year (US/Canadian Students).
  • Course of study is for either an MD or a DO degree.
  • Personal Health Coverage is or is not in effect while the student is away from school.
  • Malpractice is or is not in effect while student is away from school.
  • If your school is under the auspices of the New York State Education Department please say whether or not the student has completed New York State Infection Control Training.
  • Student is approved to take this elective for credit or not for credit.
