Making an Appointment

If your child, adolescent, or young adult has been diagnosed with cancer, you can call our Department of Pediatrics to make an appointment at Memorial Sloan-Kettering. When you call, please have the following details available:

• Personal information: Child’s name, date of birth, and Social Security number as well as names, addresses, and contact information for parents or legal guardians.
• Reason for visit: Primary site of disease or diagnosis if known.
• Primary medical insurance carrier: Our Patient Financial Services staff works closely with insurance carriers if authorizations or out-of-network approvals are required. We will obtain authorizations prior to appointments.
• Healthcare provider: Name and contact information for the child’s primary physician and for the physician or medical facility that has treated the current illness (if available).
