If your child, adolescent, or young adult has been diagnosed with cancer, you can call our Department of Pediatrics to make an appointment at Memorial Sloan Kettering. When you call, please have the following details available:

  • Personal information: Child’s name, date of birth, and Social Security number as well as names, addresses, and contact information for parents or legal guardians.
  • Reason for visit: Primary site of disease or diagnosis if known.
  • Primary medical insurance carrier: Our Patient Financial Services staff works closely with insurance carriers if authorizations or out-of-network approvals are required. We will obtain authorizations prior to appointments.
  • Healthcare provider: Name and contact information for the child’s primary physician and for the physician or medical facility that has treated the current illness (if available).
