The clinical program at Memorial Sloan Kettering Cancer Center (MSK) began in 2007 with the pharmacy department’s recognition for the need of advanced clinical training in pharmacy practice. The program has successfully expanded from two adult clinical specialists to more than 30 specialists covering multiple adult and pediatric services within leukemia, bone marrow transplant, lymphoma, multiple myeloma, thoracic, neuro-oncology and phase I services. Clinical pharmacy specialists also cover additional services including infectious disease, geriatrics and pain and palliative care.
Our clinical pharmacy specialists serve on multidisciplinary teams as experts in the therapeutic use of medications and act as liaisons between the department of pharmacy and physicians, nurses, and other disciplines. Additionally, they are actively involved in clinical research, guideline development, and contribute to national organizations essential to their fields.
With the breadth of experience from within our department, our clinical preceptors offer residents direction, leadership and support throughout the residency year with clinical and research responsibilities. Our preceptors are dedicated to the success of each resident and ensuring that the residents training and education will prepare them for clinical and administrative opportunities upon program completion.

Sherry Mathew, PharmD, BCOP
Manager, Pediatric Clinical Pharmacy Services
PGY2 Pediatric Oncology Residency Program Director
Sherry Mathew serves as the Residency Program Director (RPD) for the PGY2 Oncology Residency Program (Pediatric-Focused) at Memorial Sloan Kettering Cancer Center. She was born in New York but raised in Texas, and received her Doctor of Pharmacy degree in 2006 from The University of Houston. She completed an ASHP accredited PGY1 Pharmacy Practice Residency at Parkland Health and Hospital System in Dallas, TX. Dr. Mathew then went on to complete an ASHP accredited PGY2 Oncology Pharmacy Residency at Memorial Sloan Kettering Cancer Center (MSK). Dr. Mathew began her career at MSK in 2008 as a clinical pharmacy specialist in bone marrow transplant and leukemia where she precepted PGY2 oncology residents along with pharmacy students. In 2013 Dr. Mathew transitioned into pediatric oncology, as Manager of Pediatric Clinical Pharmacy Services and PGY2 Oncology Residency Program Director. In addition to her responsibilities as the RPD, Dr. Mathew serves as the clinical preceptor for the neuroblastoma and sarcoma outpatient rotations. Dr. Mathew is board certified in oncology pharmacy. Clinical interests include guideline development, clinical informatics support for pediatric oncology patients, and clinical research in pediatric oncology and stem cell transplant. Additionally, she serves as an active member of the Hematology/Oncology Pharmacy Association.

Lisa Modelevsky, PharmD, BCOP
Manager, Clinical Pharmacy Services
PGY2 Adult Oncology Residency Program Director
Lisa Modelevsky serves as the Residency Program Director for the PGY2 Adult Oncology Pharmacy Residency at Memorial Sloan Kettering Cancer Center (MSK). She was raised in the Chicago area and received her Doctor of Pharmacy degree in 2012 from Midwestern University - Chicago College of Pharmacy. She then went on to complete an ASHP accredited PGY1 Pharmacy Residency with the North Shore LIJ health system in New York and a PGY2 Oncology Pharmacy Residency at Boston Medical Center. Dr. Modelevsky began her career at MSK as a Clinical Pharmacy Specialist with the neuro-oncology service in 2014 and served as a clinical preceptor for both inpatient and outpatient neuro-oncology rotations. In 2022 she transitioned to the PGY2 Adult Oncology Residency Program Director and clinical pharmacy manager for the BMT, lymphoma, and multiple myeloma services. Additionally, Dr. Modelevsky serves as the clinical preceptor for the breast medicine outpatient rotation. She is board certified in oncology pharmacy (BCOP) and is a contributing author for BCOP education. Dr. Modelevsky has extensive clinical research experience primarily focusing on optimizing treatment for patients with central nervous system lymphomas and high-grade gliomas. She has also presented at national meetings including the annual conferences for the Society of Neuro-Oncology and the Hematology/Oncology Pharmacy Association (HOPA).

Raymond J. Muller, MS, RPh, FASHP
Associate Director, Division of Pharmacy Services
PGY2 Medication Use Safety Residency Program Director
Raymond J. Muller is Associate Director of Pharmacy Services at Memorial Sloan Kettering Cancer Center in New York City with an operational pharmacy budget of over $850 million. His responsibilities include assisting in the directorship of a 448-member pharmacy division, specifically the pharmacy’s drug policy management programs; training and education, and medication safety. He is the Residency Program Director for the PGY2 pharmacy residency in medication-use safety. He is an active member of the center’s Pharmacy & Therapeutics committee; the Hospital Quality Assessment & Improvement committee; and various chemotherapy practice committees and medication management task forces. He chairs the pharmacy’s quality programs and serves as the head of research pharmacy, funded by the NCI core grant. He has delivered over 300 presentations at local, regional and national educational programs. His numerous publications have appeared in BMJ, AJHP and Journal of Clinical Oncology.

Nina Cohen Orsini, PharmD, BCPS-AQID
Clinical Pharmacy Specialist, Infectious Diseases
PGY2 Infectious Diseases Residency Program Director
Nina Cohen Orsini is the PGY-2 Infectious Diseases residency program director (RPD) at Memorial Sloan Kettering Cancer Center. She received her doctor of pharmacy degree from the Arnold and Marie Schwartz College of Pharmacy and Health Sciences at Long Island University in Brooklyn, NY. Dr. Cohen Orsini completed a PGY1 Pharmacy Practice residency at Brookdale University Hospital in Brooklyn, NY and Infectious Diseases Pharmacy residency at The University of Rochester Medical Center, in Rochester NY. She has been working at MSKCC since 2005 as an Infectious Diseases Clinical Pharmacy Specialist. Dr. Cohen Orsini has many years of experience in infectious diseases pharmacotherapy and antibiotic stewardship, and has been a preceptor and mentor for residents, students and fellows. Dr. Cohen Orsini is board certified in pharmacotherapy with added qualifications in infectious diseases.

Wiktoria Bogdanksa, PharmD
Clinical Pharmacy Specialist, Thoracic Medical Oncology
Wiktoria Bogdanska serves as a Clinical Pharmacy Specialist for the Thoracic Oncology department at Memorial Sloan Kettering Cancer Center (MSK). She is originally from Connecticut and received her Doctor of Pharmacy from the University of Connecticut in 2019. She completed an ASHP accredited PGY1 Pharmacy Practice Residency at UConn Health John Dempsey Hospital in Farmington, CT followed by a PGY2 Adult Oncology Pharmacy Specialty Residency at Memorial Sloan Kettering Cancer Center in 2021. Dr. Bogdanska looks forward to becoming a board certified in oncology pharmacy and plans to serve as a preceptor and mentor for future oncology residents. Her clinical interests include teaching, targeted therapies, and precision medicine.

Manpreet Boparai, PharmD, GCP, BCACP, BCGP
Clinical Pharmacy Specialist, Geriatrics
Manpreet Boparai serves as a Geriatric- Oncology Clinical Pharmacy Specialist at Memorial Sloan Kettering for over 10 years and has been serving as a residency preceptor since the initiation of the residency program at MSK. Born in Germany and raised in India and New York, Dr. Boparai, graduated from St. John’s University with a PharmD, and then went on to complete a specialized residency in geriatrics at Duke University and the DVAMC in North Carolina. She provides consultations to older adults in the Geriatrics Clinic for medication management. Dr. Boparai works with the interprofessional team to ensure that the medications prescribed for the patients contribute to the best possible health outcomes and to avoid, as possible, drug interactions and other medication-related adverse outcomes. She is committed to advancing healthcare literacy in her patients as well as in the community. She enjoys teaching and has held faculty positions at the university level. She has presented and taught courses locally and nationally about health and wellness for older adults. She is well published in journals and books about her work in medication management and polypharmacy in older adults with cancer.

Michael Buege, PharmD, BCOP
Clinical Pharmacy Specialist, Lymphoma
Michael Buege serves as a clinical pharmacy specialist for the adult lymphoma service at Memorial Sloan Kettering. He is originally from Wisconsin and received his PharmD from Drake University in 2016. Dr. Buege completed PGY1 and PGY2 Oncology residencies at The University of Texas MD Anderson Cancer Center in Houston, Texas, and joined the lymphoma team at MSK in 2018. In 2021 Dr. Buege was selected by HOPA for sponsorship in the ACCP MeRIT research training program. His clinical and research interests include mantle cell and T-cell lymphomas, multiple myeloma, opportunistic infections, and bioinformatics.
Kristin M. Carotenuto PharmD, BCPS
Clinical Coordinator, Medication Reconciliation
Kristin Carotenuto is the Pharmacy Clinical Coordinator for the Medication Reconciliation program at Memorial Sloan Kettering Cancer Center. Kristin was born in New Jersey and received her Doctor of Pharmacy degree from the Ernest Mario School of Pharmacy at Rutgers, The State University of New Jersey in 2003. After practicing as a pharmacy manager in community pharmacy for five years, Dr. Carotenuto joined MSK to launch the pharmacist driven medication reconciliation program. She is committed to optimizing patient care and serves on various hospital committees including The National Patient Safety Goals Task Force, Pharmacy QA, Pharmacy Operational Excellence Team, Reporting to Improve Safety and Quality Reading Group and Residency Advisory Committee for the PGY-2 Medication Use Safety program. Dr. Carotenuto is board certified in pharmacotherapy. She has trained many pharmacists in medication reconciliation and is a preceptor for the PGY-2 Medication Use Safety residency. Dr. Carotenuto is dedicated to serving the poor and marginalized and has participated in medical missions to South Africa and Madagascar where she lectured to pharmacists and oncologists.
Amelia Chan, PharmD, MBA, BCOP
Manager, Drug Policy Management and Quality & Safety
PGY2 Medication Safety Residency Program Manager
Amelia Chan has over 25 years of experience in health system pharmacy serving in various positions including clinical and leadership roles to advance pharmacy practice. She currently manages Drug Policy, Pharmacy & Therapeutics, and Quality & Safety for the Division of Pharmacy Services. She is responsible for drug policy development and administration, formulary management, institutional guidelines development, medication use evaluation and quality and safety improvement. She is the manager of PGY2 Medication Safety Residency Program and serves as preceptor of PGY2 Oncology Pharmacy residents.
She serves as Vice Chair of Pharmacy Quality Assessment Committee and is an active member of Pharmacy & Therapeutics Committee, Executive Safety Committee, Chemotherapy Practice Committee, and Joint Commission Medication Management Task Force. She is a member of American Society of Health-System Pharmacists (ASHP) and a member of Hematology/Oncology Pharmacy Association (HOPA) for which she served as Chair and Vice Chair of Publication Committee in 2010 and 2009.

Ryan J. Daley, PharmD, BCOP
Clinical Pharmacy Specialist, Leukemia
Ryan Daley serves as a Preceptor for the PGY2 Oncology Residency Program (Adult-Focused) at Memorial Sloan Kettering Cancer Center. He was born in Syracuse, NY, and received his Doctor of Pharmacy degree from The State University of New York at Buffalo School of Pharmacy and Pharmaceutical Sciences. He completed an ASHP accredited PGY1 Pharmacy Practice Residency at Fletcher Allen Health Care in Burlington, VT, and went on to complete an ASHP accredited PGY2 Oncology Pharmacy Residency at The University of Texas MD Anderson Cancer Center. Dr. Daley began his clinical career at Fletcher Allen Healthcare in 2004 as a clinical pharmacy specialist in oncology where he precepted PGY1 residents and pharmacy students. In 2013 Dr. Daley began his career at Memorial Sloan Kettering Cancer Center as a Clinical Pharmacy Specialist in Adult Lymphoma & Leukemia, eventually focusing solely on Leukemia. Dr. Daley is board certified in oncology pharmacy, is a member of the MSK Pharmacy Protocol Review Committee, and is a participating member of the Pharmacy Committee of the national cooperative investigational group, The Alliance for Clinical Trials in Oncology.
Clinical Pharmacy Specialist, Lymphoma
Phuong Dao serves as a Clinical Pharmacy Specialist for the lymphoma team at Memorial Sloan Kettering Cancer Center (MSK). She is originally from California and received her Doctor of Pharmacy degree in 2014 from the University of Illinois at Chicago. Dr. Dao completed an ASHP accredited PGY1 Pharmacy Practice Residency and PGY2 Pharmacy Oncology Residency at Boston Medical Center. After completing residency, she practiced as a clinical pharmacy specialist for adult medical oncology at Massachusetts General Hospital, and then returned to Boston Medical Center as a clinical pharmacy specialist for outpatient hematology and oncology. She began her career at MSK in 2018 and serves as a preceptor for the inpatient and outpatient lymphoma rotations.
Clinical Pharmacy Specialist, Pediatric Bone Marrow Transplant
Nicole Daukshus serves as a clinical pharmacy specialist with the pediatric bone marrow transplant team at Memorial Sloan Kettering Cancer Center. She is from New Jersey and received her pharmacy degree from the Ernest Mario School of Pharmacy at Rutgers University in 2017. After pharmacy school, she went on to complete her PGY1 residency at Hackensack University Medical Center in New Jersey and subsequently went on to complete her PGY2 Pediatric Oncology Residency at MSK.
Clinical Pharmacy Specialist, Lymphoma
Raymond DeMatteo serves as a Clinical Pharmacy Specialist for the Lymphoma department at Memorial Sloan Kettering Cancer Center (MSK). He is originally from upstate New York and received his Doctor of Pharmacy from LIU Brooklyn in 2018. He completed an ASHP accredited PGY1 Pharmacy Practice Residency at Northwestern Memorial Hospital in Chicago, IL followed by a PGY2 Oncology Pharmacy Specialty Residency at MSK. Dr. DeMatteo started as a lymphoma specialist in 2020, and works both in the inpatient and outpatient setting. His clinical interests include precision medicine, T-cell lymphoid neoplasms, and teaching.

Lauren DeRespiris, PharmD, BCOP
Clinical Pharmacy Specialist, Bone Marrow Transplant
Lauren DeRespiris serves as a clinical pharmacy specialist for the Adult Bone Marrow Transplantation Program at Memorial Sloan Kettering Cancer Center (MSK). She grew up on Long Island, received an undergraduate degree in Communications & Business from New York University in 2009, then decided to pursue pharmacy as a career. Dr. DeRespiris received her PharmD from Rutgers University in 2015, then went on to complete an ASHP accredited PGY1 Pharmacy Practice residency at NYU Langone Medical Center. She recently completed an ASHP accredited PGY2 Oncology Pharmacy Residency at the Hospital of the University of Pennsylvania in Philadelphia. Dr. DeRespiris began her career as a clinical pharmacy specialist at MSK in 2017 in bone marrow transplantation. Her professional interests include mentoring pharmacy students and residents as well as clinical research.

PGY2 Oncology Preceptor, Pharmacy Practice and Operations
Brian del Corral serves as a residency preceptor for the PGY2 Oncology Residency Programs at Memorial Sloan Kettering Cancer Center. He was born and raised in Bergen County New Jersey and received his Doctor of Pharmacy degree in 2009 from Rutgers University – Ernest Mario School of Pharmacy. Brian began her career at MSK as a staff pharmacist in the main campus chemo pharmacy which provides chemotherapy for both inpatients and outpatient clinics. In 2013, Brian transitioned to a Pharmacy Coordinator roll to help facilitate chemo pharmacy and clean room operations. In 2015, Brian was promoted to Pharmacy Manager, taking over responsibility for all chemotherapy at main campus, including both pediatrics and adults. In addition to his responsibilities as a pharmacy manager, Brian takes a lead role working with pharmacy informatics, bridging the gap between operations and informatics for many projects and initiatives. Brian is an enthusiastic and forward-thinking manager, always looking for innovative ways to break from the status quo and make improvements to pharmacy practice.

Stephen Eng, PharmD, BCOP
Clinical Pharmacy Specialist, Bone Marrow Transplant
Stephen Eng serves as a Clinical Pharmacy Specialist for the Adult Bone Marrow Transplantation Program at Memorial Sloan Kettering Cancer Center (MSK). He was born and raised in Fresh Meadows, NY and began his pharmacy career at MSK in 2014 as part of the pharmacy student intern program. He received his Doctor of Pharmacy degree in 2018 from St. John’s University in Queens, NY. Following pharmacy school, Dr. Eng completed an ASHP accredited PGY1 Pharmacy Residency at Hackensack University Medical Center in Hackensack, NJ followed by a PGY2 Adult Oncology Pharmacy Residency here at MSK. In 2020, Dr. Eng began his career at MSK as a clinical pharmacy specialist in bone marrow transplantation. His professional and clinical interests includes mentoring, precepting pharmacy students and residents, and chimeric antigen receptor T-cell therapy.
Tami Guitaud, PharmD
Clinical Coordinator, Pediatric Oncology
Tami Guitaud serves as the preceptor of the PGY2 Pediatric Oncology Pharmacy Operations rotation at Memorial Sloan Kettering Cancer Center. She was born and raised in NYC, then moved to Columbus, Ohio to attend college and be with her sister. Dr. Guitaud graduated from The Ohio State University and received both her Bachelor of Science in Pharmacy in 2001 and her Doctor of Pharmacy degree in 2005. The first 12 years of her career she practiced as an adult oncology pharmacist and an investigational oncology pharmacist. She then joined MSKCC in 2013 and began practicing as a pediatric oncology pharmacist. She now serves as the clinical coordinator of operations for pediatric oncology pharmacy services at MSKCC. In this role she works closely with all disciplines and departments as a liaison to improve and provide quality care for our pediatric oncology patients being treated at main campus and the MSKCC regional sites.

Jillian Gutierrez, PharmD, BCOP
Clinical Pharmacy Specialist, Adult Leukemia
Jillian Gutierrez serves as a leukemia clinical pharmacy specialist at Memorial Sloan Kettering Cancer Center. She was born and raised in Silver Spring, MD and graduated from University of Maryland, Baltimore School of Pharmacy in 2018. She went on to complete an ASHP accredited PGY1 residency at Penn State Health Milton S. Hershey Medical Center in Hershey, PA in 2019. She then completed her Adult Oncology PGY2 residency at Memorial Sloan Kettering Cancer Center in 2020 and stayed on to begin her career as a clinical pharmacy specialist for the leukemia service. Dr. Gutierrez’s professional interests include clinical research focused on acute and chronic leukemias as well as precepting and mentorship for pharmacy residents and students.

Stephen Harnicar, PharmD, BCOP
Manager, Clinical Pharmacy Services & Research
Stephen Harnicar is a Clinical Pharmacy Manager at Memorial Sloan Kettering Cancer Center (MSK). He received his PharmD degree in 2006 from Duquesne University in Pittsburgh, PA. He completed an ASHP accredited PGY1 Pharmacy Practice Residency at The Johns Hopkins Hospital in Baltimore, MD and continued to an ASHP accredited PGY2 Oncology Pharmacy Residency at MSK. Post residency, Dr. Harnicar joined MSK as a clinical pharmacy specialist practicing in leukemia and bone marrow transplant which included responsibilities as a preceptor for both the PGY2 adult oncology residents and pharmacy students. In 2014, Dr. Harnicar took on a role as clinical manager for the Department of Pharmacy with a focus on the management of the Pharmacy Department’s clinical research goals. Dr. Harnicar currently serves on the IRB as well as having an active role in departmental QA initiatives. His clinical interests include hematologic malignancies, pharmacokinetic, and pharmacogenomic research; and has spoken internationally on these topics.

Charlene C. Kabel, PharmD, BCOP
Clinical Pharmacy Specialist, Adult Leukemia
Charlene Kabel serves as a leukemia pharmacy specialist at Memorial Sloan Kettering Cancer Center. She was born and raised in Buffalo, NY. She completed her Doctor of Pharmacy in 2015 from Duquesne University in Pittsburgh, PA. Dr. Kabel completed an ASHP accredited PGY1 residency at St. Joseph’s/Candler Hospital in Savannah, Georgia in 2016. She then completed her Adult Oncology PGY2 residency at MSKCC in 2017. Dr. Kabel started her career as an adult leukemia pharmacy specialist and continues in this position since 2017. Her research interests include outcomes in patients diagnosed with therapy-related myeloid neoplasms and lymphoid malignancies. She serves as a PGY2 preceptor, PGY2 resident mentor, and student preceptor for Duquesne University. Dr. Kabel is board certified in oncology pharmacy.

Adam Kahleifeh, PharmD
Clinical Pharmacy Specialist, Neuro-Oncology
Adam Kahleifeh serves as a clinical pharmacy specialist with the Neuro-Oncology service at Memorial Sloan Kettering Cancer Center (MSK). He grew up in Kentucky and received his Doctorate of Pharmacy from the University of Kentucky College of Pharmacy in 2019. He completed his ASHP accredited PGY1 at Baptist Health in Louisville, Kentucky. Dr. Kahleifeh then went on to complete his ASHP accredited PGY2 Adult Oncology Residency at MSK. He currently rotates between both inpatient and outpatient services. His professional interests include teaching, mentorship, and clinical research.

Amber C. King, PharmD, BCOP
Clinical Pharmacy Specialist, Adult Leukemia
Amber King serves as a leukemia pharmacy specialist at Memorial Sloan Kettering Cancer Center. She was born and raised on Long Island, NY. She received her Doctor of Pharmacy degree in 2013 from Northeastern University in Boston, MA. She completed an ASHP accredited PGY1 Pharmacy Residency at UT MD Anderson Cancer Center in Houston, TX and an ASHP accredited PGY2 Oncology Pharmacy Residency here at Memorial Sloan Kettering Cancer Center. Dr. King began her career at MSK in 2015 as a clinical pharmacy specialist in leukemia. Dr. King serves as a clinical preceptor for the inpatient and outpatient leukemia rotations for pharmacy residents. Her clinical interests include outcomes research in myeloproliferative neoplasms and lymphoid leukemias, guideline and treatment algorithm development for leukemias, investigational protocol development, and mentorship of pharmacy residents.

Lauren Koranteng, PharmD, BCPS, CPE
Clinical Pharmacy Specialist, Supportive Care Service
Lauren Koranteng serves as the Clinical Pharmacy Specialist at the Memorial Sloan Kettering Cancer Center in Manhattan, New York. She obtained her undergraduate degree from Mount Holyoke College and her Doctor of Pharmacy Degree from Temple University. Following pharmacy school, Dr. Koranteng completed an ASHP accredited PGY1 residency at the University at Buffalo/Niagara Hospice. She then went on to complete an ASHP accredited PGY2 Pain Management and Palliative Care Residency at Johns Hopkins Hospital. She is also a clinical preceptor for the Pain Management and Palliative Care rotation. She is a Certified Pain Educator (CPE) and is also board certified in pharmacotherapy.

Binni Kunvarjee, PharmD, BCOP
Clinical Pharmacy Specialist, Pediatric Bone Marrow Transplant
Binni Kunvarjee is a clinical pharmacy specialist with the pediatric bone marrow transplant team at Memorial Sloan Kettering Cancer Center (MSK). She is originally from the northern New Jersey area and received her Doctorate of Pharmacy from the Ernest Mario School of Pharmacy at Rutgers University in 2015. She completed her ASHP accredited PGY-1 at Hackensack University Medical Center in New Jersey, then went on to complete her PGY2 Pediatric Oncology Residency at MSK. Dr. Kunvarjee began her career as pediatric bone marrow transplant pharmacy specialist at MSK in August 2017 and currently rotates between inpatient and outpatient services. Her professional interests include resident/student mentorship, guideline development, and clinical research in the pediatric oncology setting.

Josiah D. Land, PharmD, BCOP
Clinical Pharmacy Specialist, Thoracic Medical Oncology
Josiah Land serves as the clinical pharmacy specialist for the Thoracic Oncology Service at Memorial Sloan Kettering Cancer Center. He was born and raised in Lake Charles, LA and received his Doctor of Pharmacy from the University of Louisiana at Monroe College of Pharmacy in 2013. After pharmacy school, he completed a PGY1 Pharmacy Residency at The University of Texas MD Anderson Cancer Center in 2014 followed by a PGY2 Adult Oncology Pharmacy Residency at Memorial Sloan Kettering Cancer Center in 2015. Dr. Land is board certified in oncology pharmacy and serves as the thoracic oncology preceptor for the adult oncology pharmacy residency program at Memorial Sloan Kettering Cancer Center. Dr. Land has served as a resident mentor and research project preceptor for the adult oncology pharmacy residency. He also periodically serves as faculty/content expert for continuing education programs in association with Creative Educational Concepts, Inc.
Director, Chemotherapy Practice and Investigational Drug Service
Melissa Lee-Teh is the Director for Chemotherapy Practice and Investigational Drug Service at Memorial Sloan Kettering Cancer Center (MSK). She received her BS in Pharmacy from Arnold & Marie Schwartz College of Pharmacy at Long Island University and her Doctor of Pharmacy degree from University of Kansas. Dr. Lee-Teh has over 20 years of pharmacy experience in the health care system and currently oversees pharmacy chemotherapy operations across MSK’s inpatient and outpatient areas. She also serves as an investigator on several MSK research protocols and leads pharmacy investigational drug services for both adult and pediatric services. She is an active member on numerous MSK committees, including Chemotherapy Practice, Pharmacy & Therapeutics, Joint Commission Medication Management, and Pharmacy Quality Assessment.

Andrew Lin, PharmD, BCOP
Clinical Pharmacy Specialist, Bone Marrow Transplant
Andrew Lin is a clinical pharmacy specialist on the adult BMT service who is board certified in oncology pharmacy. He was born in Pittsburgh, PA but grew up all over the East Coast. He received his Doctor of Pharmacy degree in 2009 from the University of North Carolina Eshelman School of Pharmacy at Chapel Hill. He completed an ASHP accredited PGY1 Pharmacy Practice Residency at Carolinas Medical Center in Charlotte, NC. Dr. Lin continued his training by completing an ASHP accredited PGY2 Oncology Pharmacy Residency at the University of Washington Medical Center/Seattle Cancer Care Alliance. He began his career at MSK in 2011 as a clinical pharmacy specialist initially with the lymphoma and leukemia services before transitioning to the bone marrow transplant service. Dr. Lin has been a preceptor and mentor for resident and student training and has served as the research mentor for several residency projects.

Dazhi Liu, PharmD, BCOP, MS
Clinical Pharmacy Specialist, Early Drug Development service
Dazhi Liu serves as the clinical pharmacy specialist of the Early Drug Development service at Memorial Sloan Kettering Cancer Center. Dr. Liu was born and raised in China. She received her Doctor of Pharmacy from the University of Iowa College of Pharmacy in 2012. Following pharmacy school, she completed an ASHP accredited PGY1 Pharmacy Practice Residency at Cincinnati Children’s hospital Medical Center and a PGY2 Pediatric Oncology Pharmacy Specialty Residency at Memorial Sloan-Kettering Cancer Center (MSKCC). Dr. Liu is board certified in oncology pharmacy. Dr. Liu began her career as a pediatric oncology clinical pharmacy specialist at MSKCC. She then joined the Early Drug Development Service, a service focused on conducting early phase oncology trials at MSKCC. Dr. Liu has many years of experience in clinical service, precepting PGY2 oncology pharmacy residents, and participating in cutting-edge clinical trials. Dr. Liu has special interest in precision medicine and biomarker-driven global oncology trials.

Morgan Mastin, PharmD
Clinical Pharmacy Specialist, Pediatric Bone Marrow Transplant
Morgan Mastin serves as a clinical pharmacy specialist with the pediatric bone marrow transplant team at Memorial Sloan Kettering Cancer Center (MSK). She is originally from Western New York and received her Doctor of Pharmacy degree from Wegmans School of Pharmacy at St. John Fisher College in 2019. She completed her PGY-1 at Johns Hopkins All Children’s Hospital in Florida, then went on to complete her PGY-2 Pediatric Oncology Residency at MSK. She currently rotates between both inpatient and outpatient services. Her professional interests include cellular therapy, precepting/mentorship, and patient education.

Lane Nguyen, PharmD, BCPPS
Clinical Pharmacy Specialist, Pediatric Intensive Care
Lane Nguyen serves as the Pediatric Intensive Care Unit (PICU) Clinical Pharmacy Specialist at Memorial Sloan Kettering Cancer Center. He was born in Vietnam and grew up in the State of Iowa. Dr. Nguyen received his Doctor of Pharmacy degree in 2016 from The University of Iowa College of Pharmacy. He completed an ASHP accredited PGY1 Pharmacy Practice Residency at The University of Iowa Hospitals and Clinics. Dr. Nguyen then went on to complete an ASHP accredited PGY2 Pediatric Pharmacy Residency at The University of Iowa Stead Family Children’s Hospital. In 2018, Dr. Nguyen joined Memorial Sloan Kettering Cancer Center as a member of the pediatric clinical pharmacy team. Clinical interests include critical care medicine, infectious diseases, pain and sedation, and mentoring students. Additionally, he serves as an active member of the Pediatric Pharmacy Advocacy Group (PPAG) and Society of Critical Care Medicine (SCCM).

Kate Reichert, PharmD, BCPPS
Clinical Pharmacy Specialist, Pediatric Hematology/Oncology
Kate Reichert is a pediatric oncology clinical pharmacy specialist at Memorial Sloan Kettering Cancer Center. She was born and raised in New Jersey and received her Doctor of Pharmacy from the University of Rhode Island College of Pharmacy in 2013. Following pharmacy school, she completed an ASHP accredited PGY1 Pharmacy Practice Residency and PGY2 Pediatric Pharmacy Specialty Residency at Children’s Hospital Colorado in Aurora, CO. Dr. Reichert began her career as a clinical pharmacy specialist in pediatric oncology at Texas Children’s Hospital in Houston, TX. In 2018, Dr. Reichert joined the pediatric clinical pharmacy team at Memorial Sloan Kettering Cancer Center in New York City. Dr. Reichert is board certified in pediatric pharmacy.

Krisoula Horiates Spatz, PharmD, BCOP
Clinical Pharmacy Specialist, Pediatric Hematology/Oncology
Krisoula Horiates Spatz serves as a pediatric oncology clinical pharmacy specialist at Memorial Sloan Kettering Cancer Center (MSK). She was born in Texas but raised in New Jersey, and received her Doctor of Pharmacy degree in 2013 from the Ernest Mario School of Pharmacy of Rutgers University. She completed an ASHP accredited PGY1 Pharmacy Practice Residency at the Penn State Milton S. Hershey Medical Center in Hershey, PA. Dr. Spatz then went on to complete an ASHP accredited PGY2 Oncology Pharmacy Residency (Pediatric-Focused) at MSK. Dr. Spatz began her career at MSK in 2015 as a clinical pharmacy specialist in pediatrics. She also serves as a clinical preceptor to PGY2 oncology residents as well as pharmacy students in the areas of inpatient pediatric oncology and outpatient leukemia/lymphoma and neuro-oncology. Dr. Spatz is board certified in oncology pharmacy.

Samantha N. Steiger, PharmD, BCIDP
Clinical Pharmacy Specialist, Infectious Diseases
Samantha Steiger is a Clinical Pharmacy Specialist in Infectious Diseases at Memorial Sloan Kettering Cancer Center. She received her Doctor of Pharmacy degree from the University of Connecticut in 2016. She completed a PGY-1 Pharmacy Practice residency at Hartford Hospital in Hartford, CT and a PGY-2 Infectious Diseases Pharmacy residency at Massachusetts General Hospital in Boston, MA. In 2018, Dr. Steiger joined Memorial Sloan Kettering Cancer Center, where she is also a member of the Antimicrobial Management Program.

Sarah Stump, PharmD, BCOP, BCPS
Clinical Pharmacy Specialist, Adult Leukemia
Sarah Stump serves as a clinical pharmacy specialist for the adult leukemia service at Memorial Sloan Kettering Cancer Center. A Tennessee native, she received her Bachelor of Science degree in Biochemistry and Cellular & Molecular Biology from the University of Tennessee and received her Doctor of Pharmacy degree in 2016 from the University of Kentucky. She completed an ASHP accredited PGY1 Pharmacy Practice Residency and PGY2 Hematology/Oncology Pharmacy Residency at the University of North Carolina in Chapel Hill, NC. Her clinical interests include teaching and mentorship, myeloid leukemias, and cardio-oncology.

Jennifer Thackray, PharmD, BCPS, BCPPS
Clinical Pharmacy Specialist, Pediatric Hematology/Oncology
Jennifer Thackray is a Pediatric Oncology Clinical Pharmacy Specialist at Memorial Sloan Kettering Cancer Center (MSK) in New York, NY. Dr. Thackray earned her Doctor of Pharmacy degree from The University of Oklahoma College of Pharmacy in 2009. She completed an ASHP-accredited PGY1 Pharmacy Practice Residency (2010) and a PGY2 Pediatric Pharmacotherapy Specialty Residency (2011) at the University of North Carolina Health Clinics in Chapel Hill, NC. Dr. Thackray is board certified in pharmacotherapy and pediatric pharmacotherapy. Dr. Thackray joined the MSK Department of Pharmacy in 2011, initially practicing in the pediatric oncology inpatient setting. Over her tenure at MSK, Dr. Thackray has assisted in developing pediatric clinical services in the outpatient leukemia clinic and the outpatient sarcoma clinic. Her clinical practice includes rotating between inpatient oncology, outpatient leukemia and outpatient neuro-oncology clinical services. Currently, Dr. Thackray serves as a clinical preceptor in these areas, a PGY2 resident mentor and a research mentor for PGY2 resident research projects. Dr. Thackray’s clinical interests and research efforts include pediatric oncology supportive care measures (e.g., chemotherapy-induced nausea and vomiting, venous thromboembolism, menses suppression, etc), pharmacogenetics and clinical guideline development within international organizations. Dr. Thackray is an active member of the American Society of Health-System Pharmacists (ASHP), American College of Clinical Pharmacy (ACCP), Children’s Oncology Group (COG), Clinical Pharmacogenetics Implementation Consortium (CPIC), Hematology/Oncology Pharmacy Association (HOPA) and Pediatric Pharmacy Advocacy Group (PPAG).

Grashma Vadakkel, PharmD, BCOP
Clinical Pharmacy Specialist, Bone Marrow Transplant
Grashma Vadakkel serves as a Clinical Pharmacy Specialist for the Adult Bone Marrow Transplant service at Memorial Sloan Kettering Cancer Center (MSKCC). She is originally from Coral Springs, Florida and received an undergraduate degree in Biology from the University of Florida in 2013. She then went on to complete her Doctor of Pharmacy in 2018 from the University of South Florida College of Pharmacy. Following pharmacy school, Dr. Vadakkel completed an ASHP accredited PGY1 Pharmacy Practice Residency at Nebraska Medicine in 2019. She then went on to complete an ASHP accredited PGY2 Adult Oncology Pharmacy Residency at MSKCC in 2020 and joined the Adult Bone Marrow Transplant group post residency training. She looks forward to becoming board certified in oncology and plans to serve as a preceptor for the inpatient and outpatient bone marrow transplant rotations.

Katherine S. Vicinanza-Rickard, PharmD, BCPS
Clinical Coordinator, Drug Policy Management
Katherine Vicinanza-Rickard is a Drug Policy preceptor for the PGY2 Medication-Use Safety & Policy Residency Program at Memorial Sloan Kettering Cancer Center. She also serves as a preceptor for the PGY2 Pediatric Oncology & Infectious Disease Residency Programs for a rotation in Medication Event Analysis, Management and Error Prevention Strategies Dr. Vicinanza-Rickard is originally from Queens, New York, and received her Doctor of Pharmacy degree from MCPHS University-Boston Campus. She completed her PGY-1 Pharmacy Residency at the Northport Veterans Affairs Medical Center, and then went on to complete a PGY-2 Pharmacy Residency in Medication-use Safety at Memorial Sloan Kettering Cancer Center. Dr. Vicinanza-Rickard is currently a Clinical Coordinator in Drug Policy Management where she implements revisions in the formulary database, participates in RISQ events analysis, provides drug information to pharmacists and clinicians, updates and manages parenteral guidelines, and contributes to projects related to clinical, quality, safety, and financial outcomes of drug use. Dr. Vicinanza-Rickard is board certified in pharmacotherapy.

Taylor Weis, PharmD, BCOP
Clinical Pharmacy Specialist, Leukemia
Taylor Weis serves as a Clinical Pharmacy Specialist for the leukemia team at Memorial Sloan Kettering Cancer Center. She is originally from Concord, North Carolina and received her Doctor of Pharmacy degree in 2017 from the University of North Carolina Eshelman School of Pharmacy at Chapel Hill. She completed an ASHP accredited PGY1 Pharmacy Practice Residency at Michigan Medicine in Ann Arbor, Michigan in 2018. She subsequently completed a PGY2 Oncology Pharmacy Residency at Michigan Medicine in 2019. She began her career at MSK in 2019 as a leukemia clinical pharmacy specialist and looks forward to precepting both the inpatient and outpatient adult leukemia rotations. Her clinical interests include teaching and outcomes research in acute lymphoid and myeloid malignancies.

Stacy Wong, PharmD, MSHS
Manager, BAIC/Kimmel Pharmacy Operations
Stacy Wong serves as the preceptor of the pharmacy operations rotation in the Medication Safety Residency at Memorial Sloan Kettering Cancer Center. She is also a member of the Residency Advisory Committee for the residency. Dr. Wong was born and raised in Wilmington, Delaware and received her Doctor of Pharmacy from Rutgers, the State University of New Jersey in 2008. She decided to try the city life and moved to the Big Apple after graduating to start her career at Memorial Sloan Kettering Cancer Center. She has staffed in various areas including inpatient areas, pediatric outpatient chemotherapy, and adult outpatient clinics. In January 2018, Stacy received her Master of Science in Health Sciences in Regulatory Affairs from the George Washington University.
Fuwang Xu, PharmD, BCOP, BCGP
Clinical Coordinator, Quality Assessment & Improvement
Fuwang Xu serves as a preceptor for both the Medication-Use Safety & Policy Residency Program, and the Adult Oncology Residency Program at Memorial Sloan Kettering Cancer Center for a longitudinal rotation in Medication Event Analysis. Fuwang received his Doctor of Pharmacy degree from The Ohio State University and completed both his PGY1 Pharmacy Practice Residency and PGY2 Oncology Pharmacy Residency at Northwestern Memorial Hospital in Chicago. After that, he moved to NYC and completed another year of training in Medication-Use Safety at MSK. Fuwang is currently a Pharmacy Clinical Coordinator in Quality Assessment & Improvement. He actively works on departmental QA initiatives and contributes to various projects related to medication policy, safety, and regulatory compliance.

Shirley Qiong Yan, PharmD, BCOP
Clinical Pharmacy Specialist, Pediatric Hematology/Oncology
Shirley Yan is a pediatric clinical pharmacy specialist in the inpatient Hematology/Oncology service as well as the Leukemia/Lymphoma and Neuro-oncology outpatient services. Pursuing her love of math, science and statistics; the first collegiate degree that Shirley earned was a Bachelor of Science degree in Computer Science from the University of California Riverside. After completing her degree, she worked as a computer scientist until deciding to apply her passion to the field of Pharmacy. Shirley returned to scholastics and completed her Pharm.D. at the University of California San Francisco where she was accepted into the Pharmaceutical Science Pathway and worked under Dr. Janel Long-Boyle PharmD, PhD. Her areas of research included pediatric cancer therapeutics, pharmacokinetics and pharmacodynamics. She then completed an ASHP accredited PGY1 Pediatric Pharmacy Practice Residency at Children’s Hospital of Orange County (CHOC) in Orange, California. Due to her interest in Pediatric Oncology and clinical research, Dr. Yan then traversed the states to participate in the ASHP accredited PGY2 Pediatric Oncology at Memorial Sloan Kettering Cancer Center (MSK). Upon completion of her residency, she joined MSK as a pediatric oncology clinical pharmacy specialist, where in addition to her clinical duties she also precepts pharmacy students, PGY2 Pediatric Oncology residents and PGY2 Medication Safety residents. Dr. Yan is board certified in oncology pharmacy and her clinical interests include teaching, guideline development, and clinical research in the pediatric oncology population.
